L’intolleranza all’istamina (HIT) è il termine per quel tipo di intolleranza alimentare che include una serie di reazioni indesiderate come risultato dell’istamina accumulata o ingerita. Nelle linee guida tedesche del 2017, gli specialisti tedeschi e svizzeri preferiscono il termine “reazioni avverse all’istamina ingerita.
Questa è una condizione causata da uno squilibrio tra l’istamina rilasciata dal cibo e la capacità dell’organismo di degradare tale quantità. È accompagnato da una ridotta attività dell’enzima DAO, che porta ad un aumento della concentrazione di istamina nel plasma e all’insorgenza di reazioni avverse. La carenza di DAO predispone un certo sottogruppo della popolazione ad intolleranza all’istamina.
Quali sono i sintomi dell’intolleranza all’istamina?
- Nausea
- Stanchezza
- Arrossamento del viso
- Mal di testa/ emicrania
- Crampi addominali
- Orticaria
- Prurito
- Ipertensione
- Vertigini o capogiri
- Tachicardia o aritmie
- Ansia
- Crampi addominali
- Congestione nasale, starnuti, difficoltà respiratorie
- Difficoltà ad addormentarsi
- Gonfiore addominale
- Ciclo mestruale anormale
Quali possono essere le cause di ridotta attività della DAO?
La ridotta attività dell’enzima DAO, può essere di origine genetica, patologica, carenziale, farmacologica ma anche data da disturbi gastrointestinali funzionali:
- Sensibilità al glutine non celiaca (NCGS)
- SIBO (proliferazione batterica del piccolo intestino)
- Leaky gut (iper-permeabilità intestinale)
- Disbiosi
- Malattie intestinali infiammatorie croniche (IBD)
- Mutazione o predisposizione genetica
- Carenze vitaminiche
- Alimenti che inibiscono l’attività della DAO (alcool, bevande energetiche, tè)
- Assunzione di alcuni farmaci
La diagnosi di intolleranza all’istamina è di difficile interpretazione (come abbiamo visto i sintomi sono molto comuni ad altri disturbi) e controversa perché non c’è ancora un metodo diagnostico comune convalidato. Ultimamente in letteratura scientifica sono state proposte da alcuni autori, delle tabelle con criteri e analisi di riferimento da seguire, per fare un’ eventuale diagnosi di intolleranza all’istamina. (Di questo ne parlerò in maniera più’ specifica nel mio libro che sto scrivendo a 4 mani, per portarvi tutte le ultime informazioni scientifiche sull’intolleranza all’istamina). Tra i vari criteri c’è il test di provocazione con la dieta. Si farà una “challenge” con dieta povera di istamina a confronto di una dieta contenente istamina per valutare la sintomatologia che servirà per fare diagnosi.
Una volta sicuri che il problema ai vostri sintomi è l’intolleranza all’istamina è stato dimostrato che una dieta povera di istamina associata ad un’integratore di enzima DAO migliora notevolmente il quadro sintomatologico. Il medico valuterà anche l’assunzione di farmaci antistaminici.
Non dobbiamo scordarci di vedere il quadro in maniera completa e integrata, quindi il percorso nutrizionale sarà rivolto, oltre che a ridurre ed eliminare la sintomatologia, anche a ripristinare la funzionalità intestinale e dell’intero organismo con lo scopo di riequilibrare il benessere totale.
